Really. It's because<br />

<li>I was too lazy to go to the DFA main consular office in Parañaque City, which is far from my place. Satellite offices only accepted renewals then, and</li>
<li>I was feeling discouraged due to reading from several blogs that they were not allowed inside the office because their documents were not enough to prove their identity. Back then I only had my birth certificate and no government-issued ID.</li>
</ol>
Nowadays, satellite offices accept even new applications, and just recently, one opened up near where I live. I also now have my <a href="http://proud2bcurvy.blogspot.com/2013/07/applying-for-sss-umid-initial.html" target="_blank">UMID</a>.<br />
<br />
Fortunately, DFA has an online <a href="http://www.passport.com.ph/" target="_blank">passport appointment system</a> and they do not allow walk-ins except for senior citizens and children one year old and below. I registered for the 10:30am time slot and filled up, printed the application form - which is required to enter their office.<br />
<br />
<i><b>Time in</b>: 10:00am </i><br />
<br />
Procedure:<br />
<ol>
<li>Line up according to time slot. The first person in queue is given a cardboard with printed time slot to hold up.</li>
<li>Have your application form ready. The guards need to see this so they can let you enter the office.</li>
<li>Proceed to the Information Counter. They will check your application form and give you a number depending on your purpose (new application, renewal). Senior citizens and children one year old and below are given a separate set of numbers.</li>
<li>Grab a seat and wait for your number to be shown on the TV on the right (near the Processing area). There are more than enough seats.</li>
<li>When your number shows up, proceed to your assigned window in the Processing area. They will inspect your documents and verify your identity.</li>
<li>After verification, proceed to the Cashier to pay the passport fee.</li>
<li>Again, grab a seat and wait for your number to be shown, this time on the TV on the left (near the Encoding area).</li>
<li>When your number shows up, proceed to your assigned window in the Encoding area. They will type in your details, have you check them for errors, and take your biometrics (thumbprints and photo).</li>
<li>(Optional) Proceed to Courier area if you want to have your passport delivered to you. They use <a href="http://www.lbcexpress.com/" target="_blank">LBC</a> as their courier.</li>
</ol>

<span style="font-size: small;"><span style="font-family: "Courier New",Courier,monospace;">8:00am - finished filling up the form and figuring out where to line up. I swear this was the most confusing part.</span></span><br />

<li><span style="font-size: small;"><span style="font-family: "Courier New",Courier,monospace;">Bring all the valid IDs you can think of. You'll never know how strict/overbearing the employee assigned to verify your identity will be. In my case though, he wasn't strict and was really following the list of requirements.</span></span></li>
<li><span style="font-size: small;"><span style="font-family: "Courier New",Courier,monospace;">If you brought your own E-6 form, make sure that it is not outdated. Better yet, get a new one from the branch and fill it up. Some people brought outdated forms this morning and unfortunately those were not accepted.</span></span></li>
<li><span style="font-size: small;"><span style="font-family: "Courier New",Courier,monospace;">Do not wear any jewelry near the face like earrings, nose rings, necklaces. They will ask you to take them off before taking your photo anyway.</span></span></li>
<li><span style="font-size: small;"><span style="font-family: "Courier New",Courier,monospace;">Do not wear contact lenses, for the same reason as tip #2.</span></span></li>
<li><span style="font-size: small;"><span style="font-family: "Courier New",Courier,monospace;">Do not be afraid to ask. Guards and employees assume that you already know what you are doing once you're inside the branch. They don't help out if you don't ask.</span></span></li>

<br />This particular volunteer is male, of average height and with
black glasses. I'm describing him since I don't have his name. I first
encountered him a Thursday last week. He approached me and I said "No"
for about 3 times before he backed off and said "Thank you". I found it
annoying but I let it go.<br />
<br />The following day, I saw him again and as soon as he saw me (I
was amidst maybe more than 10 people whom he just let pass without
asking at least one of them to donate), I waved him off but he went a
bit near me and said, "Tagong-tago Ma'am ah.", loosely translated as
"You're really hiding Ma'am.". This has made me feel threatened and to
be honest I am a bit scared of going through the underpass again later
but I have no choice as it's the only way I can get to the other side of
Ayala since it's the one nearest my office (Ayala Life FGU).<br />

2. <u>Bring water, food, a fan, and hankies.</u> There's a drugstore and a coffee shop that opens earlier than the mall but it would be better to bring your own food so that you wouldn't have to leave your place in the line.</div>

3. <u>Only 1 valid ID is needed.</u> I don't know about the other centers though. I only used my birth certificate, and I didn't even have to show it.<br />

requirements for Voluntary Permanent Disconnection (VPD):<br />

- Completion of the contract or settlement of the applicable<br />
Pre-Termination Fee (PTF) if still within contract computed as: Monthly<br />
Service Fee x number of remaining months under lock-in period<br />
<br />
- Settlement of outstanding balance <br />
- Completely filled-out and signed form for the said request <br />
- Valid ID of the registered subscriber <br />
<br />
You may also send a representative to transact on your behalf with the<br />
following documents: <br />

- Letter of Authorization (signed by the subscriber) <br />
- Photocopy of the subscriber's valid ID <br />
- Representative's original valid ID <br />
<br />
Meantime, we have forwarded request to our support group for waiving of<br />
Pre-Termination Fee. Please note, that this is still subject for<br />
approval. We will advise you of any updates once we received feedback<br />
from our support group.<br />
